Best Practices and Gap Analysis, Advancing High-Reliability and Safety Culture in Healthcare

There are five main principles that exist in the High-Reliability Organizations (HROs): being preoccupied with failure, not simplifying, operational sensitivity, resiliency commitment, and deference to expertise. These attributes are critical in the development of a strong culture of safety and quality in healthcare. My healthcare organization partially conforms to the HRO best practices, with the excellent commitment of the executive to safety, an open incident reporting system, and the multidisciplinary quality improvement (QI) programs. Root cause analysis, leadership rounds, and safety briefs are indicative of focus on failure and respect to expertise. But, there are still some loopholes to be filled in terms of consistency and standardization of cross-functional operations of the organization especially in terms of communication between care units.

The organizational needs assessment findings indicated that there was a severe gap in quality associated with handoff communication. The extent to which structured tools, including the SBAR (Situation, Background, Assessment, Recommendation) format are inconsistently used, is a factor in medication errors, incomplete documentation, and higher readmission rates. This variance with the best practices of HRO demonstrates a lack of sensitivity to operations and insufficient resilience response to high-risk care transition. According to research, the high reliability in healthcare facilities can be sustained based on the existence of structured communication and psychological safety (Rotteau et al., 2022).

Evidence-based interventions that can be used to address this gap include handoff communication standardization and improvement of feedback. The use of the integrated handoff protocol that incorporates both SBAR and bedside shift reports and electronic checklists will significantly enhance accuracy and accountability. Effective communication behaviors are strengthened with simulation-based training and competency validation on a regular basis. Moreover, the establishment of daily safety huddles and Leadership WalkRounds with instant feedback will lead to a culture of learning, which upholds the transparency and responsiveness (Finn et al., 2024). These are strategies that are in line with best practices that recognize the need to engage the leaders and focus on continuous process improvement (Patrician et al., 2024). The key performance indicators, including handoff compliance rates, incident-report closure time, medication error reduction, and subscale scores of the Hospital Survey on Patient Safety Culture (HSOPSC) will be used to measure the progress and will be subject to iterative refinements using the Plan-Do-Study-Act (PDSA) model. To sum up, the culture of safety and the general quality of healthcare can be reinforced by coordinating organizational activities with the HRO principles based on the structured communication, real-time feedback, and evidence-based leadership interventions.
